About the role

Salary: £30,060 per annum + excellent benefits

Hours: Full time, 37 hours per week

Contract: Fixed term (2 years)

Location: Based in Winsford, working across GP Practices in Winsford & Northwich (with some home visits and home working)

We’re seeking a passionate and skilled Disability Rights Adviser to join our Community Advice Team. This is a unique opportunity to help shape an innovative outreach service designed to reach people with disabilities and long-term health conditions who might otherwise go without support.

What You’ll Do:

  • Provide high-quality advice and advocacy on disability rights and related welfare issues

  • Work in GP practices, undertake home visits, and support clients who cannot access services independently

  • Develop structured referral pathways in collaboration with GP staff

  • Help embed this project into local primary care systems

  • Drive improvements in access and outcomes for disabled people in the community

Who You Are:

  • You bring strong experience in delivering social welfare advice, with a sound knowledge of disability rights and benefit entitlements. You’re organised, self-motivated, and comfortable working across varied settings. Empathy, resilience, and strong communication skills are essential.

  • Desirable: Experience with Citizens Advice or working in primary care environments.
